Recommended US or UK Virtual Private Server (VPS) or Dedicated Server Providers

Can anyone recommend a good Virtual Private Server (VPS) or Dedicated Server provider located either US or UK with good connectivity, good reputation, no or low set-up fee, pay monthly. I've looked at Verio UK & US but am put off by the pay-quarterly minimum term. I'm looking for an actual provider, not a reseller. It's been a while since I've been in the market for such a service so am not too au fait with who the best providers now are. Can anyone venture recommendations? Or equally I'd like to hear non-recommendations. Thanks...
